What is TEAMWorks? 
TEAMWorks, modeled after Kaiser Permanente's successful Diabetes Morning, is a half-day diabetes program presented by the DpCP and the peer review organization, Quality Partners of Rhode Island.  At TEAMWorks, groups of patients with diabetes receive key diabetes self-management information from a team of diabetes educators (nurse, dietitian, and pharmacist) and their physician. The results of lab tests (HbA1C, lipid profiles, and tests for microalbuminuria), obtained prior to the program date, are available to the team during the program. 

Patients attend small group skill-building sessions with a nurse and a dietitian, receive one-on-one medication counseling with a pharmacist, and have a one-on-one consultation with their physician. Referrals are made for dilated eye exams, dental care and diabetes outpatient education. The TEAMWorks program is offered to adults with diabetes.

What is TEAMWorks?
TEAMWorks is a half-day educational program designed to help people better manage their diabetes.  A team of diabetes experts along with the patient's doctor will work together to help the person with diabetes make small changes in their life-style, to help improve their health.

Who is on the "team"?  
The participant, a dietitian, nurse, pharmacist, and the  participant's doctor.  Everyone works together to develop a plan to help the participant best manage their diabetes.

What will the participant learn?  
The participant will receive several tips on diabetes and diabetes care including:

  • Making good food choices
  • Estimating food portion size
  • Proper use of medications
  • Exercising safely
  • Proper foot, eye, and dental care
  • Blood glucose monitors and supplies
  • New products on the market
  • Third party reimbursement
  • Nutrition and diabetes information resources

What does TEAMWorks cost?
There is no charge beyond the usual co-payment for an office visit at the participating doctor's office.

How do I get referred to TEAMWorks?  If you are 18 years or older you are eligible for the program.  You should discuss this with your physician or contact the DPCP at (401)222-4851
